Community Based Services

Community Based Services offers specialized services for individuals in our catchment area who have been diagnosed with a severe and persistent mental illness. By providing these services at the community level, we can assist participants to remain in the community, thus reducing the need for inpatient psychiatric care at a state hospital.

A multi-disciplinary staff works together to make the various Community Based Services successful. Our CBS Paraprofessional staff is the strength of the Community Based Services system. Working in the various components of our program, the CBS Paraprofessional strives to maximize the strengths of the individual and the individual's ability to live independently in the community.

Our goal is to reduce hospitalization frequency and duration by assisting severe and persistently mentally ill adults to integrate into the community by providing individualized quality services, including case management, educational groups, crisis intervention and supported employment. Strengthened through psychiatric, medical and liaison services, an individual will learn to identify and link with community resources to become and remain a productive member of society.
