The care plan is delivered through a collaborative process involving staff, community health care providers, the resident and his/her family. RALM has several facilities in the Sandhills section of North Carolina. Counties served include: Moore, Duplin, Sampson, Cumberland, Guilford, Craven, Lenoir, Wake and Wayne.
